Conducive Conditions Inspection

An inspection of the moisture and pest-friendly conditions around and under a structure: wood touching soil, earth-filled porches, mulch against siding, gutters and downspouts, grading, plumbing leaks, the vapor barrier and ventilation in an under-floor area, slab and basement conditions, standing water, firewood and debris, and plants touching the walls. Each condition found gets a location, a recommendation and a photo, and moisture readings are typed by location. Built for pest control companies to hand to the owner.

Fill this in and download the finished PDF. Your answers stay on this device as you type. We ask for an email address once, at the download.

This form records the conditions only. To record evidence of termites and other wood destroying insects as well, use: Wood destroying insect inspection

This records conditions that encourage pests and moisture. It is not a wood destroying insect report, a mold assessment, a structural evaluation or a code inspection.

If you work under another person's certification, put theirs here and your registration in the notes. Licensing rules differ by state — check your state lead agency.

Wood and Soil at the Foundation

Wood, fill and debris that give insects a hidden way in, walked all the way round.

Codes commonly expect wood siding and framing to sit several inches above finished grade.

Fill packed against the foundation hides it and can carry insects straight to the framing.

A conducive conditions inspection is a walk around and under a building looking for the things that make it easy for termites, carpenter ants, wood-boring beetles, moisture-loving pests and wood decay to move in: wood touching soil, mulch heaped against the siding, downspouts emptying at the foundation, a torn vapor barrier, a leak under the bathroom, firewood stacked against the wall. Most of them are fixed by the owner, not with a pesticide. This template gives every condition its own line, and anything present gets a location, a recommendation and a photo, so the owner knows exactly what to change. Fill it in here and download a PDF without an account, or speak while you walk, and the document writes itself.

Why conditions get their own report

Most termite and moisture-pest problems start with water and wood in the wrong place. A treatment can protect a house for years, but if the gutters still dump water at the slab and the mulch is still six inches up the siding, the house stays attractive and the treatment is working against the odds.

Writing the conditions up separately does two things. It gives the owner a short list they can act on, and it records what the company told them on that date. When activity appears later at the very spot where a condition was noted, that record matters to everyone.

Present means it goes on the list

Each condition is answered Not present, Present or Could not check. Present opens three things: where it is, what the owner should do about it, and a photo. Those become the findings on the report, so the owner reads a list of specific places, not a general warning about moisture.

Could not check is an honest answer and deserves a note. A buried foundation, a locked access panel or a basement full of boxes limits what anyone can say about that part of the building.

Under the floor, on the slab, in the basement

Choose the foundation types at the start and only the sections that apply will open. Under-floor areas get the vapor barrier, the vents or dehumidifier, standing water, plumbing leaks, wood scraps on the soil and condensation. Slabs get cracks, open joints and plumbing penetrations. Basements get dampness and the walls hidden behind finishes or storage.

Moisture readings are typed by location, with the material and a plain-words level, so a reading in a sill plate can be compared with the same spot next year. Wood that stays wet enough to support decay, often cited as around 20 percent and above on a wood moisture meter, is the reading worth writing down twice.

What this form does not decide

This is a pest control company's record of conditions, not a structural, mold or building code inspection, and not a wood destroying insect report. Where a condition needs a roofer, a plumber, a grading contractor or a structural engineer, say so in the recommendation and leave the diagnosis to them.

Common questions

What are conducive conditions?

Conditions around or in a building that make it easier for pests, especially termites and other wood destroying insects, to arrive and survive: moisture, wood in contact with soil, food sources such as wood debris, and hidden routes in such as foam or soil covering the foundation.

Is this the same as a WDI inspection?

No. A WDI inspection records evidence of wood destroying insects as well as conditions, and in a real estate sale may need an official form. This template records conditions only, and works alongside a WDI inspection or a treatment.

Who fixes the conditions on the list?

Usually the owner or a contractor they hire. Each recommendation can say who, and the summary list at the end has a column for it. Some pest control companies offer moisture or exclusion services as well; record those as recommendations too.

Do I need a moisture meter?

Not for every item, but readings in the under-floor area, at sill plates and around leaks make the report far more useful and let you compare visits. Note the meter and the scale you used.

Can this be done at every renewal visit?

Yes. Many companies repeat it at the yearly renewal of a termite contract, so the owner sees the same list each year, with the fixed items gone.
